Paperwork! Unfortunately there are a lot of items to review and sign to get started- then it is smooth sailing.

Please complete -

• Prospective Employees - so you understand what is expected.
• Direct Deposit Auth - so we can pay you!
• W-4 Form - so the Govt knows how we are paying you!
• Non-compete Agreement - because we will be sharing proprietary information with you.
• Confidentiality & Photo Release- you are in a position of trust and may be photographed.
• Live Scan Request- you will need to get a background check.

The Workers Compensation information you can review at your leisure. If you have any prior injuries please let us know so we can be mindful of your needs.
